As an employee (or potential employee) at an institution/facility (e.g., kindergarten, daycare center, child welfare

facility, youth shelter, youth activity center, medical institution), I consent to an inquiry into my criminal record for sex

offenses in accordance with Article 56 of the Act on the Protection of Children and Youth Against Sex Offenses, Article

25 of the Enforcement Decree of the same Act, and Article 8(3) of the Enforcement Regulations of the same Act. I also

consent to a comprehensive inquiry into my criminal record for child abuse-related offenses in accordance with Article

29-3 of the Child Welfare Act and Article 26-5 of the Enforcement Decree of the same Act.
